Important Information for Sports Field User ( 18k/1p)

Sportsfields require a permit and fee for use.  Provided here is access to the application forms, facility inventories and other information you'll require to complete the application process.

How to Apply

• Sportsfield Application Form ( 29k/2p) - Legal size paper 8 1/2" x 14"
• Payment Authorization Form ( 29k/1p)
• Tournament / Special Request Application Form ( 29k/2p)
• Milne Park Application Form ( 34k/2p)
• Milne Park Map ( 404k/1p)
  
  Please send your completed application form(s) by mail to:
  Town of Markham, Attention: Sportsfield Permits Coordinator, 101 Town Centre Blvd., Markham, ON, L3R 9W3.
  
  Payment Options: Cash (at the Civic Centre Building - Unionville Entrance, bring invoice with you), OR Visa or Mastercard ("Payment Authorization Form", can be faxed to (905) 475-4710), OR Cheque (payable to "Town of Markham", contract number must be written on the cheque).
  
  Please Note: Cheques will not be accepted if booked less than 30 days before your event.
