PDF Form Instructions

To fill out a form:

1. Download the form to your hard disk by right-clicking on the link and select "save link as" or "save target as" from the dropdown options. Make a note of the location on your drive and the name of the file being saved so you can locate it later.
2. Use Acrobat 7.0 (or later) to open the file.
3.

Select the hand tool. Position the pointer inside a form field and click. The I-beam pointer allows you to type text.

4. After completing the appropriate form fields, print the form, sign it and sent it by mail, fax or in-person (See each form for submission instructions). Most forms are 8 1/2" x 11" (letter size) except where indicated.
5. We suggest you print an extra copy for your records.

Important: You cannot save the data in a form using the Save or Save As commands. These commands save the form itself, but not the data entered into the form fields.

Application forms from the Planning Division are available in PDF file format below. Simply click on the relevant file format to download. Once downloaded you can review the forms, complete them and submit them to the Planning and Development Department at the Development Services Counter at the Markham Civic Centre (Thornhill entrance)

We are open for School programs Wednesday through Friday from September through June. Guided Tours can be booked during two daily time slots: either 10 am to noon or 12:30 - 3:30 pm. Teachers can combine Generic topics with exhibition-related topics or request a custom tailor programs to meet specific needs!

Please book your tour at least 14 days in advance. A booking request form and a 50% deposit is required in order to confirm your booking.
