To dispute your Penalty Notice you must request a Screening Review appointment within 15 days from the date of issuance by using the link below to schedule a date and time that works best for you.
*It can take up to 5 business days for your Penalty Notice to be available online.
Once you have completed this form, you will receive a confirmation email from firstname.lastname@example.org when the appointment request is approved.
If you do not receive your appointment notification within 24 hours of submission, it is your responsibility to follow up on the status of your request by calling 905.477.5530.
If you require an interpreter during your appointment, you are responsible for bringing your own interpreter. The City does not provide interpreters.
